The managed care environment harbors a partnership of mutual responsibility. We are committed to partner with you to attain optimal care, i.e. the right care at the right time. We will do our very best to work within the guidelines of your managed care insurance policy. We respectfully ask you to do the same.
We attempt to keep our fees structured to the customary level in the community.
Payment is expected at the time of service. Credit cards accepted (Visa, Discover and MasterCard).

Fee for Forms:

Due to the overwhelming volume of paperwork, there will be a $5.00 charge for any form or note that requires a physician's signature. Unfortunately, this fee has become necessary because of the significant staff and physician time needed to complete these documents. Thank you for your cooperation in this matter.
